Questionnaire
The practice has commissioned a survey to give patients a chance to feedback on their feelings about the practice, the staff, and our services.
The survey is available in both paper form, in the practice, and via the web, using the link below. Please note, that you should complete the survey AFTER you have had a consultation.
We would like to assure patients that all answers are anonymous and that individuals cannot be identified by their responses.
Once we have had the results collated, they will be made public, and we will call a meeting of the Patient Participation Group in order to discuss the results.
